Many veterans don’t seek help for themselves. Not out of pride, but often because they’re unsure what support is available or how to get started. If you're a family member, carer, or close friend, you can play an important role in helping them access the in-home care they may be eligible for.
Start With a Conversation
Gently asking how they’re managing at home can open the door. Are they struggling with medication routines, wound care, or mobility? Even small issues like frequent fatigue or trouble getting to appointments can be signs that extra support is needed.
Know What Support Looks Like
Community nursing at home can include things like:
Medication management
Health checks or chronic condition support
Help with showering or dressing
Wound care after surgery or injury
This care is delivered by qualified professionals and designed to maintain dignity, safety, and independence.
How to Help With Access
While only authorised health professionals can make a formal referral, family members can:
Book a GP appointment on their behalf
Accompany them to medical visits to raise concerns
Help communicate changes in their health

What to Expect After a Referral
Once referred, the care provider will contact the veteran (or their representative) to arrange an in-home assessment. This helps create a care plan that’s tailored to their needs, with no obligation to proceed unless they’re comfortable.
