What Our Community Nursing Services Include
We deliver a range of in-home nursing services tailored to individual health needs. The type and frequency of care depend on clinical assessments and authorised referrals from healthcare professionals. Services may include:
Wound care – For surgical recovery or chronic wounds requiring professional management.
Medication management – Including support with dosage schedules, reminders, and monitoring.
Chronic condition support – Help managing long-term illnesses such as diabetes, COPD, or heart disease.

Who Can Access These Services?
Community nursing is available to eligible veterans who hold a valid health card issued under an approved program. This includes Gold Card holders with accepted conditions. However, the decision to refer a person for community nursing must come from a qualified medical practitioner or discharge planner, based on clinical need.
It’s important to note that eligibility is not automatically granted. It’s assessed individually by the referring professional. If you're unsure whether you or your loved one might qualify, speak to your GP or healthcare provider. We are also available to answer general questions about the process.
The Referral Process: Step-by-Step
Consult with a GP or Hospital Team
A general practitioner, hospital discharge planner, or another authorised professional assesses the person’s health status and determines whether community nursing is suitable.Referral Sent to Provider
If appropriate, the healthcare provider submits a referral to a community nursing provider such as Deekay Health. The referral outlines the person’s clinical needs and care goals.Initial Contact and In-Home Assessment
Once we receive the referral, our team contacts the client or their nominated representative to arrange a convenient time for an initial home assessment.Development of a Care Plan
